refactor: improve header layout and remove Product Hunt badge

- Move documentation and X (Twitter) links from settings modal to header
- Remove Product Hunt badge from header
- Reorder header icons for better UX (docs, X, Discord, GitHub)
- Clean up settings modal header controls
